Camp Health Forms

The 2024 Camp Health Form, and Medicine Authorization form are mandatory documents for every camper. We also require copies of up to date immunization records and copies of your insurance cards.  Finally we encourage all participants to submit a copy of the COVID 19 Vaccination Card. We do NOT reuse documents for returning campers. Please upload the forms to your account or you can Return forms via email to Syracusesoccercamps@gmail.com no later than two weeks before the start of camp.
